2 STEPS BEFORE ARRIVAL: Complete the following two steps for a smooth check-in for your appointment!

STEP 1 – LOGIN TO THE PATIENT PORTAL

The Patient Portal will allow you to schedule an appointment and complete the patient interview required before each visit.

If you are a brand new patient or have not received your personalized log-in to access the patient portal, please call us at 720-870-2828. It is important that you access your patient portal and fill out your patient interview prior to your visit.

Please note, when you click the link above, you will be redirected to our online scheduling service, Revolution PHR.

STEP 2: INTAKE FORM & PRIVACY SUMMARY

This form is short and sweet. As a medical office, we must confirm with every patient at every visit that we protect your personal information as required by law and share how your information is used.

Signing this form online in advance is one less thing we must do at check-in.

All patient visits including adjustment and dispensing of glasses will need to have a scheduled appointment. We will not be able to accommodate “drop-in” patients at this time.
